Patient Information

Our practice welcomes new patients. We accept most major insurances. Please check with your insurance provider for further information. Kettering Health Medical Group Primary Care - Sugarcreek is located at 6438 Wilmington Pike, Suite 100 in Dayton, Ohio in the Sugarcreek Health Center. Our office is open Monday through Thursday from 8:00 a.m. to 5:00 p.m., and Friday from 8:00 a.m. to 3:00 p.m.

To make an appointment, call the office at (937) 558-3840. New patients should arrive 15 minutes early to complete the registration health forms and documents. If you have an emergency and need to speak to a member of Kettering Health Medical Group Primary Care - Sugarcreek after office hours, please call the Medical Society at (937) 463-1656. If you receive a registration packet in the mail prior to your appointment, please complete the information and bring it to your appointment. It’s important to bring the following information and/or documents with you to your first appointment:

  • Consent to treat (If patient is a minor and parent/guardian is not present)
  • Government issued photo ID
  • Insurance card(s)
  • Forms for pre-op physical (if applicable)
  • All medications (including over the counter medications)
  • A list of any specialists that you see regularly

We encourage you to become familiar with the requirements of your insurance policy, including understanding your plan benefits, co-pay, referrals, pre-certification, covered facilities, and services information. We file both primary and secondary claims for our patients. Patients are required to pay all applicable co-pays and deductibles at the time of service. Patients are responsible for any non-covered service fees performed at the time of service. We will inform you of these services and ask you to complete a waiver stating the same.

Please be aware that we cannot treat any work-related conditions. Please check with your employer for their recommended worker’s care facility. We cannot bill any third party carriers for motor vehicle or personal injury cases. The patient is responsible for payment at the time of service. A receipt can be issued for reimbursement.
