This practice is accepting new patients and accepts insurance providers that cover Kettering Health physicians and practices. Far Oaks Orthopedists has several locations in the Dayton area:

Please note that not all services are available at each office, and some locations have limited hours. To make an appointment or for more information, please call our office at (937) 433-5309, Monday through Friday, 8:00 a.m. to 4:30 p.m. When you call, please have the name of your referring physician and your insurance information available. If you have already had X-rays taken or an MRI/CT scan done related to the condition for which you wish to be treated please let us know. We encourage you to bring X-rays or scans to your appointment to help provide the best quality visit. It is important for you to bring the following information with you to your appointment. Without this information, we may need to delay or reschedule your appointment:

List of medications
Films from any x-rays, MRIs, CT scans or bone scans done in the last six months
Reports for any EMGs, vascular tests or Doppler studies done in the last six months
Insurance cards
Referral from your Primary Care Physician, if required by your insurance carrier
Any co-pay required by your insurance carrier
